Housing Access Services

Housing Access Services serves clients of any age who need safe and stable housing. The services are provided on a 1:1 ratio. Clients must have been diagnosed with an intellectual or developmental disability, and be approved for services through Alta California Regional Center (ACRC). Includes two components:

  • Individual Housing Transition Services
  • Individual Housing & Tenancy Sustaining Services

Individual Housing Transition Services include:

These services provide direct support and assistance with activities and processes associated with an individual's preparation for, and transition to, housing.

  • 1

    Tenant Screening

    Conducting tenant screening and housing assessments to identify preferences, barriers to tenancy, and housing retention challenges.

  • 2

    Create Individualized Plan

    Creating an individualized housing support plan with measurable goals, strategies, and resources to address barriers including non-Medicaid services.

  • 3

    Identifying Necessary Resources

    Helping secure resources for utility setup (electricity, water, heating, etc.), essential services, pest control, and pre-move cleaning.

  • 4

    Housing Application Assistance

    Assisting the individual with the housing application process. Assisting with the housing search process including roommate selection.

  • 5

    Coordinating Resources

    Coordinating solutions for any potential unsafe conditions in the living environment before move-in. 

  • 6

    Moving Assistance

    Supporting move logistics, including lease review, utility setup, and communication with landlords.

  • 7

    Developing a Crisis Plan

    Developing a housing support crisis plan that includes prevention and early intervention services when housing is jeopardized.

What does Individual Housing & Tenancy Services look like?

This service is made available to support individuals to maintain tenancy once housing is secured. The availability of ongoing housing-related services in addition to other long-term services and supports promotes housing success, fosters community integration and inclusion, and develops natural support networks.

  • Tenant Assessment

    Early identification and intervention for behaviors jeopardizing housing (e.g., late rent, lease violations)

  • Housing Education & Training

    Education and training on tenant and landlord roles, rights, and responsibilities.

  • Interpersonal Coaching

    Coaching to build and maintain positive landlord/property manager relationships.

  • Conflict Management

    Assistance in resolving landlord or neighbor disputes to prevent eviction or other adverse actions.

  • Community Resource Access

    Advocacy and linkage to community resources to address housing risks.

  • Recertification Assistance

    Assisting the individual with the housing recertification process.

  • Individualized Support & Crisis Plan Review

    Regular review and updates to the housing support and crisis plan based on the individual's needs.

  • Tenant Training

    Ongoing training and support for lease compliance and household management.

 

Housing Access Services does not include payment for room and board. Persons receiving Health Homes or California Community Transitions services will not receive this service unless additional Housing Access Services through the waiver is necessary to maintain the consumers’ health, safety and well-being in the home and/or community.
