Individual Program Plan (IPP)
Click HERE to view the full guide on Individual Program Planning
The Individual Program Plan is a way to tell other people what kind of support you need, how you want to get that support, and what's most important to you. It's about figuring out your life goals and how to achieve them by having shared conversations between you and people who care about you, like family and friends. It is a way to talk so that everyone respects your ideas and listens to you. This will help you figure out what you enjoy and what gets in the way of living the life you want. The four-part guide linked above is designed to help individuals build a person-centered individual program plan
The IPP Program Guide walks you through...
Building Your Individual Program Plan
Think about what is important and what matters to you. Decide what you want and don't want in your life.
Before Your Planning Meeting
Fill out the decision-making checklist, determine necessary services, remember your rights.
During Your Planning Meeting
Go over your list of things to talk about, ask questions, receive assistance.
After Your Planning Meeting
Compete the meeting survey and a check-in every six months to ensure the plan is still working.
